Description: This naevus has a focus of severe atypia in the extreme "north-west". At this site there are blue-grey structures and peripheral brown dots.

History: The 62 year old man presented with a chest infection following surgery to replace his mitral valve prosthesis due to SABE vegetations. A melanoma was encountered on the right side of his right back and this lesion on the left. Histology was reported as showing a dysplastic naevus with moderate atypia with a very small focus of severe atypia
